- Public buildings, walkable spaces including parks, seating and public toilets will have been reviewed with recommendations to becoming ‘Age Friendly’.
- Age Friendly Transport and options with community focused solutions.

- People supported to connect with social networks through their GP and through Community Connectors.

- Develop a standardised process for ongoing engagement and co-production and build on these open discussions.
- Establish an Adults steering group to develop and manage action plans, ensuring Health, Social Care, Providers and older people are represented.
- Work to establish a baseline of where we are now to allow us to monitor progress on where we want to get to by 2027.
