Have you recently been in hospital?
We are collecting the experiences from people who have been discharged from hospital in the mid and south Essex area.
We will be using your feedback to improve the way patients are discharged, the services you could receive in the community and the way you are supported to ensure you have the right care to meet your needs when you get home.
We encourage anyone who has been discharged from hospital since 1 August 2024 to give us their views. The survey should take 10-15 minutes to complete, but it may take a little longer if you choose to write long responses. Anonymity
Your responses will be used by the Mid and South Essex Integrated Care System (MSE ICS) for the purposes of analysis and reporting on an anonymous basis. Any data shared with us, will NOT contain any information that could identify you. The report will be available to the public and anonymous quotes may be included.
Closing date
This is an ongoing survey so we won't be closing his survey until the end of March 2025.
