Annual Review process
This survey has been developed to gather feedback from parents/carers to help the Southend SEND Team (referred to here as the Local Authority or ‘LA’) evaluate the annual reviews of Education Health and Care Plans.
Introduction
Education Health and Care Plans are for children and young people up to the age of 25 who require special educational provision to meet their special educational needs. It is often called an 'EHCP', or EHC Plan, and they are based on a co-ordinated assessment and planning process undertaken by the LA.
The EHC plan should be reviewed every 12 months – this is referred to as an Annual Review.
Who is this survey for?
We want to hear from families that have been through the annual review process for their child or young person's EHCP since September 2021.
You can retrospectively complete the survey for the 2021-22 academic year and can then submit another response following this academic year’s annual review. We will collate the results of this survey into academic years.
What do we want to know?
The survey asks questions that will help us to understand:
- the experience of reviewing support
- the impact of support from the Education Health and Care plan
- the difference it makes to the progress of children and young people and their lives, both in and outside of education
- any gaps in services, community and leisure activities
Completing the survey
The survey should take less than 15 minutes to do. It should be completed after the annual review meeting has been held. You can either do it at the end of the annual review meeting, or a week or so after your review when you have had time to reflect.

You can also print out the Easy Read version of the survey and return to us at the address given on the document. The Easy Read version can be found on the right hand side of this page.
Who will read my answers?
The LA will read your answers to understand the range and quality of provision and support in the local area. Feedback will support improvements in service delivery and practice.
A summary of the results will be shared with Southend schools and social care and health care staff, so that improvements can be made to the way things are done. They will not know who has completed the surveys.
