  • To want to be supported to have a good and meaningful everyday life. 
  • That care and support should be person centred
  • Should have choice and control.
  • Would like to be supported to live in the community.
  • Have a choice about where and with whom they live.
  • Want good care and support from accessible local health services.
  • Want to be able to access the community to be able to enjoy leisure opportunities.
  • There should be support available to in the event of a crisis.
  • Want to be able to have volunteering and work opportunities.
  • Would like support and access to become digitally included

  1. Health and Wellbeing—Promoting healthy lifestyles for the adult population
  2. Prevention— Preventing people from going into Hospitals/care homes
  3. Housing– Deliver health, care and housing in a more joined up way
  4. Community– Enable people to remain part of and active in their communities and be digitally included.
  5. Integrated, strengths-based person-centred care–enable people to take control of their care and focus on their strengths

  • Fully accessible health services provided in several locations.
  • Individuals report that they can influence local service delivery and feel listened to
  • There is one clear provider of accessible information, advice and guidance.
  • Choice of accommodation that suits a variety of needs
  • Options available to provide support to enable individuals to remain where they are currently living.
  • A variety of volunteering and work opportunities available.
  • Access to equipment to remain independent for longer
  • Individuals only have to tell us their story once.
  • Individual’s report being able to access transport to be able to go out
  • Develop a standardised process for ongoing engagement and co-production and build on these open discussions
  • Establish an Adults steering group to develop and manage action plans, ensuring Health, Social Care, Providers and people who use service are represented.
  • Work to establish a baseline of where we are now to allow us to monitor progress on where we want to get to by 2027
  • Produce an annual action plan for 2022/23 for Living Well in Southend.
