Parent/Carer Survey - education, health and care needs assessment process
This survey has been developed to gather feedback from parents/carers to help the Southend SEND Team (referred to here as the Local Authority or ‘LA’) evaluate the education, health and care needs assessment process.
Introduction
An education, health and care needs assessment is a formal assessment undertaken by the LA in order to determine if special educational provision is required for a child or young person. This is often referred to as an EHC needs assessment or EHCNA.
Who is this survey for?
We want to hear from families that have been through the EHC needs assessment process since September 2021.
We will collate the result of this survey into academic years.
What do we want to know?
The survey asks questions that will help us to understand the experience of families during the EHC needs assessment process and whether they feel:
- fully involved in the assessment and decision-making process
- fully involved in developing their child’s Education, Health and Care plan (EHCP) if awarded
- feel other professionals are fully involved and contribute to the development of an EHCP if awarded
Completing the survey
The survey should take less than 15 minutes to do. It should be completed after the EHC needs assessment process has finished.

You can also print out the Easy Read version of the survey and return to us at the address shown on the bottom of the document. The Easy Read version can be found on the right hand side of this page.
Who will read my answers?
The LA will read your answers to understand if the process of obtaining support for children and young people is helpful or not.
Overall survey results will help to inform a review of the assessment process and support improvements in service delivery or practice.
A summary of the results will be shared with Southend schools and social care and health care staff, so that improvements can be made to the way things are done. They will not know who has completed the surveys.
